If that happens again, try a Sync reset. I haven't had a problem with the USB ports not charging devices, but when Sync does a reset, the power on those ports will cycle off and on.

Simple. The charging logs would indicate the vehicle was charged at a Tesla supercharger. If you didn't use a supercharger with a magic dock (which could be determined by the station number in the logs) the only possible option was an adapter was used.
