If i am charging at a Tesla super charging station, do I use the TESLA phone app, or the Ford one? Does it matter?
is there a cost difference?

You don't need the Tesla app.
If the charger is part of the plug and charge network, which it should be, you will just connect and charge.
Ford will then bill you when you disconnect.

This goes for all of the services. If you want to sign up/pay for a membership for a given service and get that discount you need to use the App from that service. Otherwise, you are going to be paying the non-member price.

Note: That's $12.99/mo, or $155.88/yr. Makes sense IF you charge at Tesla chargers enough to where the discount rate offsets the monthly membership charge. If you're using Tesla chargers only occasionally (like me) the monthly membership fee doesn't make sense.

Break even is around 110 kWh of energy.

You can use both. The Ford App did not play well with Tesla, but since the update both work seamless.

Per month.

About 12¢ per kWh savings. $12.99 / 0.12 = 108.25 kWh
