This is what I pulled out. Have no idea what it is or where it came from. Looks like a cover to something. Anyone know?

That looks like a pin cap. It's the plastic thing at the tip of DC pins. It must have fallen off another vehicle into the plug, then gotten shoved into your port when you plugged in somewhere.

It's a good idea to look inside the DC plug before plugging in, just in case it's damaged or there's debris inside it.
