I stopped by the dealer, and of course it worked on the first try, and worked fine again today. He said it may be because my phone is old its BT signal is erratic, and the cord I have is poor quality.
I bought a new cord, trying a USB-C instead of USB-A. I’ll report back.
A month later, I haven’t had to try the new cord. I’m using the charge-only USB for charging and BT for CarPlay so as not to confuse it.
I’m used to CarPlay systems that connect MUCH faster, and would assume there was something wrong and start trying other options. It doesn't explain why using USB for CarPlay was so spotty, but at least it’s working via BT as long as I wait (annoyingly) longer for connection than in other cars.

The car went into the dealer for the main contactor replacement (yeah, we got the alert on the dash) and it miraculously came back with the bluetooth issue resolved as well... We also noticed some changes to the software on the screen, so they must have known about this issue and fixed it.
