Our leased car is 23 mos old. The EVSE appears to have died. Is this common?
Yes, it is VERY common. Hundreds of posts across social media. Same is true for other OEM chargers.

I have a 2021 Job 1 and used the Ford charger as a level 2 exclusively for a year and a half Then it threw the yellow light and throttled back from approx 6KW to 3KW output. Yellow light said it was over heating but it was only 50 degrees in the garage. I temperature gunned the 240 vac connection, the charger and the charge port. The charger was hottest at about 85 degrees surely not overheating.
I normally left the charger plugged into the wall at all times and just connected/disconnected at the car. Doing this it would through a yellow light within minutes. I found that if I disconnected the charger from the wall it may charge for a couple hours before yellow lighting.
It took Ford 6 months to replace it such a long story you probably wouldn't believe me.
Anyway the local Ford service rep brought the replacement to my house himself. Plus Ford gave me a free extended warranty, on the whole car.
I had, before the replacement, installed an emporia charger which I am very happy with.
I have used the Ford replacement charger a few times and it will yellow light drop the charge rate down for a while then it kicks back up.
It's ok for a backup but I wouldn't depend upon it.

I have an FCCS that I’ve been happy with. I got it when (actually before) I got my 2021

Thanks -- I'm getting the picture. We're leasing the car and hope that the dealer will replace the unit at no cost. We plan to buy the car . . . & get a Grizzl.

I'm currently using a Grizzl-e Mini (mobile charger) as my main charger & leave the Ford mobile charger in the car. The Grizzl-e Mini cable is a little thicker & 25' long vs 20' for the Ford. I've been happy with the Grizzl-e so far (early days - it's only been a couple of weeks).

It's simple - if you're within 3 years, they will replace it under warranty. If not, they won't. Whether you are leasing or buying is irrelevant to the warranty.

So this thread has me curious, when you folks take the Ford supplied charger on the road, where are you finding 240v outlets of matching plug configuration?

That helps. I'm hoping the dealer is willing to pull a unit out of a Mache on the lot if they don't have one in stock. We live at the end of a 3mi long dirt road in a rural area. We need to charge at home. I'm typing this reply from the showroom waiting for the Gen'l Manager.

I have been using my FMC since I bought my Mach E as my primary L2 charger. I have owned my Mach E for almost 4 years. I have replaced the FMC twice, both under warranty after the unit consistently was drawing less than 5kw. Ford dealership replaced the unit without issue after confirming the loss of FMC performance.

Yup. Mine made it about 18 months, and was replaced under warranty. I now have a Wallbox, and keep the Ford one for backup or travel. The Ford unit looks exactly like my neighbor's Lexus charger. Probably the low cost Manufacturer solution.

It has both a 120V plug and a 240V plug included. You just remove the one you don't want, and replace it with the compatible plug.

I got mine replaced after 23 mos. We live in a remote location20 mi from the closest fast charger. We'll buy a backup charger near the end of the warranty though we are thinking of extending the warranty.

That is another drawback with the FMC: It only works at 120V/12A or 240V/32A. If you happen across a 240V/30A outlet like the ones at RV parks, you are out of luck. I bought a Tesla Mobile unit because I needed it to be compatible with a NEMA 10-30R outlet. 🤔🐩

EDIT: I should note that at the time I bought my Tesla Mobile Connector it was significantly less expensive than the FMC. It was so much less expensive that had I not already had a Lectron 48A Tesla L2 adapter, it'd still have been quite a but less expensive to buy both than the FMC alone. Not sure about pricing now though. 🤷‍♂️🐩
