Several weeks ago I got a warning that there was a Front Camera Failure. I had to leave the car at the dealer for the day (took a10 days to get an appointment) and after many attempts to resolve the issue they determined it needed a module replacement. Waited another week for the new module to arrive and several days to get another appointment. Left the car there for day but the technician could not get the module to accept the programming. They called Ford tech support but couldn't get an answer that day so i took the car home. A week later they were told the new module was probably defective and they needed to order a new one. Its 10 more days and they have not yet received the new module. Its now nearly a month, still no resolution and I am not confident that the new module is going to fix this but we'll see.

The problem is that without a front end camera there is no front end collision avoidance, I can't use cruise control ( not even basic cruise control), and automatic high beams is non functional too. None of those are deal breakers but the lack of cruise control has been a pain on som long drives the past two weeks.

Has anyone else had an issue like this?

Yes, I've had many many and still have issues with it.

On the day of arrival I experienced it once, it went away by turning the car on and off, second day it happened again when I was going home from work, and rebooting the car did not remove it...

Since then I've had it at the dealership who tried to update the module, but it seems there is no updates available, and now I'm getting the error more and more frequently. The other day I was getting the warnings every 2 minutes while driving for an hour ..

Ford told my dealer the module needed to be replaced. Hopefully the second time is the charm. They should get new one this week. I'll let you know if it fixes the problem.

Fingers crossed, my dealership reached out two days ago to get it back in the shop for some debugging as they expect it might be faulty hardware after talking with some other dealerships

Before you take a mache infor Servive for anything you need to disconnect the 12 volt battery for a minute. It is like rebooting your I phone, computer, internet, plus whatever.

My wife's car is having this problem now. Never logged a fault before we took it in for the recalls and the 21P22 update. When they were loading the new module programs the FCM threw a fault they weren't able to clear. Like yours, they determined it needed a new FCM. Tried one and it also had the code, so they're trying a second one. We'll see I guess.

Not to depress you but that started in mid November. It's now mid Feb and the problem has not been fixed yet. Let me give you a quick summary. Maybe this will help your dealer

  • First trip to the dealer - They tried to update the software - FAIL
  • Second trip to the dealer a week or so later - They tried to install the new module the ordered -FAIL
  • At this point they said they were ordering another module. I got in touch with Ford because I thought doing the same thing again sounded like another FAIL
  • Third trip to the dealer - Took another week or so to order a second replacement module - FAIL again(surprise)
  • Then Ford offered to do a remote consult while they worked on the car. The holidays and Omicron came along and technicians were out sick and it took forever for them to have a loaner available so I had to wait while.
  • 10 days ago finally got a loaner and brought the car in. They took care of the sun roof windshield recall and tried to repair the camera fault with remote access from Ford -FAIL
  • Now Ford offered to send a tech to NY to work on site with the dealer. He's been here since Wed. They tried a 4th module but it also would not accept the software. Now they think they may have had the voltage set wrong when they installed the module. -FAIL
  • The current plan is to order another module (#5) and retry this again hopefully Tue if the module arrives quickly and reinstall with the proper voltage.
So as of this moment its been 3 months and still a work in progress. I'll send an update when the problem is finally resolved and I'm back in my car in stead of this gas guzzling Ford Explorer loaner car.

We have the car back with the front camera issue apparently resolved.

The second module came in Monday afternoon and they installed it that afternoon.

They apparently had some issues getting it programmed--Ford Pass sent a couple of 'Pre-collison assist not available' notifications late yesterday afternoon--but the service rep called this morning saying the car was ready.

My wife (@waterbabie, the other ? reaction to your previous post ?) went and picked it up late this afternoon, and said it seemed fine on the drive home of about 30 miles.

Of course, she doesn't use ACC ? and didn't experience any situations that might have triggered pre-collision or AEB, so I can't confirm the front sensor is working properly. I'll have to drive it tomorrow just to satisfy myself that all is well.

Anyway, keep your head up; this apparently isn't an intractable problem.

Good to hear. They still have my car as they had to order another module. They’re working on it today (which I know because the car keeps sending me messages this morning about the collision avoidance not working) so hopefully fifth time is the charm. New tech is always tricky when unique new problems arise. Apparently your technician was better equipped to deal with it or perhaps Ford learned something from my fiasco that helped him. Best of luck

Same thing is happening to my car. Had the windshield recall and 21P22 done two weeks ago. Throwing the issue randomly since. Last two days, I got 5 notifications of front collision while the car was parked. ACC works randomly. I'll watch this thread before sending it to the dealer, hopefully another update resolves the issue.

So the last chapter in this story has hopefully been written. I finally got the car back yesterday. The picture below is what it looked like the day before. Total bill for this repair (just for the past 2 1/2 weeks not the prior three visits) was over $16,000 according to the service manager. Since it's under warranty the bill goes to Ford. Not sure how that works and if Ford actually pays that but that's what they are claiming.

So far everything seems to be working but I have to take the car up to the highway to make sure the cruise control is working.

When I asked what the problem was and why they had to replace the same module 5 times the explanation I got is that the part number for the module was mislabeled in the manual. They claimed the part number ended in an"e" and should have ended in an "a" or something to that affect. They blamed Ford but at this point I am not sure what to believe.

At any rate after 4 months and 4 separate visits to the service department and a total of 3 1/2 weeks without the car it seems like the saga is finally over. Hopefully they put it all back together correctly and didn't have any parts left over. Fingers crossed.

This was a recent special service message, possibly a result of your repairs:

Feb 16 2022 2021 Mustang Mach-E - Image Processing Module-A (IPMA) Programmable Module Installation (PMI) Fails / IPMA Becomes Unresponsive

Some 2021 Mustang Mach-E vehicles may exhibit an image processing module-A (IPMA) programmable module installation (PMI) fail and that the IPMA becomes unresponsive. This may be due to an electronic parts catalog (ECAT) listing error resulting in incorrect replacement IPMA service part number. The catalog will be corrected within the next catalog update cycle, approximately 30 days. For vehicles built on 26-Oct-2020 and through 05-Jul-2021 and built with 360 degree camera use Service Part Number (SPN) LJ8Z-19H405-E. Vehicles built on 26-Oct-2020 and through 05-Jul-2021 built with rear view camera only (less 360 degree) use Service Part Number (SPN) LJ8Z-19H405-D. The catalog listing is correct for vehicles built on or after 06-Jul-2021.

Sounds exactly like what the dealer said. I wish someone had figured this out before the dealer had to replace the part 5 times. Kudos to the eagle eyes person who finally discovered the problem
