I have had an intermittent issue with the adaptive cruise control seeing a phantom car in front and slowing down. When this happens and I remain hands free in blue cruise, the car throws up a red box "adaptive cruise control" with a crash icon and beeps a lot. When I disengage blue cruise and all driver assists, the dash screen shows the phantom car in front with a red line between us, indicating I am too close.

I was hoping that updating all my modules to latest software would help. Unfortunately it did not, but it did seem to increase the frequency of the issue occuring.
If you still get the phantom car thing your cruise radar may need an alignment. That procedure is in FDRS. There is vertical alignment which is checked with the front bumper cover off and using a level, and after that there is horizontal alignment which is done with FDRS
Normally the vertical alignment should not have changed from whatever the plant adjusted it to unless it's gotten an impact that's knocked it loose.
I did perform the horizontal alignment in FDRS. It didn't fix the issue. I did not look at the vertical alignment since I wasn't ready to pull the front bumper off. I'm just collecting as much data as I can so I can preset it to the dealer for them to be successful in doing a warranty repair on this issue. I really don't want to be taking the car in multiple times for them to fix this.

That is exactly what I’m seeing. My car is 8 months old and this is the first time it has occurred. Heading out today and will see if taking the plate off did anything. Are you out of warranty?

I have had the exact same issue. I took it in complete with a video as proof and they said they "repaired" it but it does it from time to time, including when I left the dealer after the "repair". If you find a solution, please let us know:

@IcemanCS Do you know what they did to "repair" it?

I've only been driving around town, but on nearly every trip on the freeway that is > 15 miles, it will occur once. I highly doubt it is a problem with the mounting being misaligned or loose. If it were, I would think the error condition would occur nearly all the time.

'INSPECT. FDRS TEST, FOUND B1578:89 AND u0565:86 IN IPMA. PER TSB 24-2087, REPROGRAM CMR TO LATEST LEVEL WITH FDRS. CLEAR ALL DTC'S AND VERIFY OPERATION." I picked it up and within a few miles of getting on highway, the same issue happened. It cannot be predicted. Sometimes it happens daily, sometimes it goes weeks without happening. Very frustrating.

It sounds like their "repair" was just to update the module software like I did. I remain worried if I take it in for them to look at it, they too won't be able to easily reproduce it. Even if they can reproduce it (or I record a video like you did), they still may not know how to fix it. Maybe they jump straight to replacing the radar module at the front of the car?

I'm doing a cost/benefit analysis of just living with the issue or going through the trouble of taking it in and seeing what they can do. My wife uses the car a lot and doesn't like it being in the shop at all. On the flip side, I paid a lot for this car, so I expect everything to work as advertised.

Do you happen to use a non-OEM front license plate bracket?

My guess is the sensor bracket is actually out of alignment. Perhaps you could stick a camera through the grille shutters to see. There are other reports of the bracket coming loose or breaking unexpectedly. It is fragile and hitting a pothole can do it.

I can confirm it's easier to see the sensor bracket with the frunk out, but it's a bit of work.

I might be jinxing it, but ever since I got PMA-24.204.8.27 OTA this problem has not occurred. Here's to hoping its fully gone.
