Is this due to the technician cost not being refunded by Ford for fixes not specifically required or just a general don't do what you absolutely don't need to do model? You can provide all of us with some insight and possibly help everybody understand the hows and whys of the dealer servicing model.

Correct. Warranty pays for the actual repair of verified defects. Much like doctors aren't a big fan of self diagnosis by Web MD, manufacturers don't go for forum diagnoses.
Whether or not the dealer will write up and attempt to fix an unverified concern varies. In a software update situation it is virtually impossible for Ford to say you never experienced the issue, so there is no exposure for the dealer to get charged back. Parts replacement is another story.

My dealer applied every SSM applicable to my car before delivery. PAAK works perfectly (among other things).
