For those who experienced this @bbachrac @flyingdutchman , I had this happen to me consistently, repeated many times, including going in Reverse up an incline while the selector was in Drive. My car was at the dealership for about 5 weeks, and the service techs there also replicated the issue multiple times. There were no error codes found, and no corresponding Technical Service Bulletins available.

What finally resolved the issue was replacing the Gear Shift Module (GSM), which they said was a recommended repair after running a number of pinpoint tests. Copying the notes from my invoice if it will help you with your dealers:

"TECH CONTACTED HOT LINE. STATED TO PROCEED DIAGNOSING SUSPECTED FAULT, TECH RAN PTS/OASIS WSM SECTION 307-05 GLOBAL CUSTOMER SYMPTOM CODE (GCSC) START/RUN/MOVE PINPOINT V, PERFORMED PPT V LEADS TO PPT F, F1-NO, F2-NO, TO F4-YES F9-NO, F13-YES. PER PPT F13 STATES TO INSTALL NEW (GSM) GEAR SHIFT MODULE FOLLOW PROCEDURES AS PER WSM SERVICE PROCEDURE 307-05"

My issue is now gone, hope this helps.
Interesting, that means the GSM failed the return-to-park function test. When the car is in drive with the belt unbuckled, opening the driver's door should automatically shift to park.

I suspected this could be a GSM bug, there have been some in the past. TSB 22-2342 dictates replacement of the GSM on vehicles built before Jan-18-2022 for a bug that allows shifting from park without pressing the brake first.

Others with the reverse issue may want to try this test:
  • Start the engine.
  • Repeat the following steps 3 times:
    • Sit in the driver seat with the seat belt unbuckled, the driver door closed, and Park selected.
    • Select Drive.
    • Open the driver door.
Does the GSM return to Park when the driver door is opened? (Should be yes)

Thanks. My dealer also said GSM. However, they did not replace the part. Instead, they "reflashed" the GSM, per "TSB 21-2405".

I have no idea whether this did, or did not, resolve the issue for me. I have not had an incident in the 3 months since the "reflash", but then I only had two occurrences in the 17 months before the reflash. I would probably feel better if they had replaced the GSM, like your dealer did, but perhaps the flash is all that was needed. Time will tell.

Then people need to do what I do whenever my car acts odd.

Take a video, then take the car to the dealership.

Nearly impossible to get anything fixed without proof. And everyone has a video camera (phone) on their person at all times.

It’s like dozens of people claiming to see the same UFO and no one thinks to film it?
That doesn't even work with Ford. Last time I took my car in I showed the service advisor what was happening in person, at the service drive. He said, "Well, I've never seen that before". He sat in the car, fiddled with it and wrote it up.

5 days later I go to pick up the car and the service order says "Technician unable to replicate, please show technician next time". And they also charged me for the technicians time.

Your situation is just an example of a horrible service center. They replicated the problem, then said they couldn’t?

Might have helped if you had a video they could show the technician. ? Not sure how your example of NOT having a video proves that videos don’t work………..

At the very least, a video should help all the deniers on the forum. (Well almost all…….. remember the Puma puddle lights??)

I'm not saying that videos aren't good. I'm just saying that blanket statements of videos leading to solutions, and dealerships actually giving a damn, is not always true. As for the forum, definitely agree that it will squash the naysayers.

Who made a blanket statement that videos will always have the dealership fix the problem? Not me!

If anything it’s people making the opposite “blanket statement,” that videos don’t help.

They can, and did, for me. I even posted the video. The dealer didn’t even attempt to recreate the issue (I’ve never even had it happen again), yet they ordered me a new screen with the video being the only evidence.

My only point? Film it when something goes wrong. It CAN (but not always) lead to a solution either from people here or at your dealership. YMMV

Now I am curious what your issue and video were about. Maybe the reason my video “worked” and yours didn’t was that there was a specific fix (service bulletin) for my issue.

My wife just called me and encountered this. Parked in the garage, was a bit close in, so turned the shifter to Reverse, and when went to back up a little, it went forward (into the shelves, damaging both the shelves and scratching the car’s paint).

Has there been any issues identified by Ford (and addresses)?

Well, it still happens. Just had my car try to go forward while in reverse, trying to back out of a parking spot. I realized what it was doing because of the ultrasonic sensors, since it was behaving and beeping the way it does when I park in my garage (inching forward until a few inches from the wall, slowly). Put it in park, then reverse again. No good. Put it in drive, then reverse. No good. Turned the car off for a few seconds, then back on and reverse. No good. Put it in park, dead half this thread, realized I had it sitting in park longer so let's try again, and poof, reverse goes backwards, no parking sensor beeping that I am too close to the car in front of me, and I can go home. Hurray! No harm, just inconvenient because of the time spent trying to fix it. Thankfully, I knew I'd seen this issue here in the forums, and have lots of practice inching forward without hitting things, so I was fine. Just... This really shouldn't still be happening.

Can you clarify a few things? So the car was detecting an obstacle, which is why it wouldn't reverse? Did you move the obstacle behind the car? Maybe a cat was back there or something? When you said it was going forward, was it just moving a couple inches to avoid the obstacle behind?

Also are you aware if you were getting rear or front obstacle beeps? A lot of people aren't aware they're different sounds, the front beeps are higher pitch.

When people say the car is moving forward, I'm not sure if that just means it's rocking a tiny amount while the car is trying to hold a position, or if it's actually wanting to move forwards multiple feet in distance.
