My car has had this error for almost 3 years. We brought it in right before warranty and they did a SW update, and it worked for a month or 2, then stopped and the warranty was over. We've dealt with it, but I have the car in at the dealership now for warranty work on the high voltage battery junction box under the 8year/100k, and had them quote me to fix this error. They said it will be an "extensive re-program" and would cost 1400 .... I dont think ive seen that as a solution on this forum, it has always been coax issue, anyone run into this ?

The tech said that they tested cameras/sensors and they worked fine . not sure how valid this is and dont want to drop 1400 dollars if a reprogram doesn't fix it.

Maybe @rugedraw can see if there are any DTCs which indicate the cause.

I am assuming that he means updating the IPMA which would take them maybe two hours to do. If it is a bad coax or camera, reprogramming/updating the IPMA will not accomplish anything.

Maybe @rugedraw can see if there are any DTCs which indicate the cause.
If he sends me then VIN, I can check the code history.

Pop off the cover under the mirror and check ye ol coax cable. Odds are it has wiggled loose. If the error pops up when you go over bumps then this is almost 100% the cause.

If you're near Omaha I'll only charge you $1399 to fix it ;)

Software does not make sense to me. My dealership also wants $ 1,400 to fix the problem by doing a seven hour software update. I reported it to NTHSB as a dangerous flaw. In no way do I believe the software just corrupted itself.

Also, see TSB 26-2021 about this. The TSB my service person quoted to me has been superseded. The new TSB says up to seven hours to fix the entire problem. My problem is I now do not trust the service department.

When I had this issue repaired under warranty on my 2021 First Edition the dealership service folks told me they had to replace the adas module (I forget it's exact name) located in the left rear quarter panel area. (this is where the coax goes to.)
They told me the reason they had to replace it was because as per the repair process they were trying to reprogram it, but that procedure kept failing. They did not replace the coax cable. The front camera has been fine since (two and a half years now).

Allof this to say, it is conceivable to me that the remedy could be software reprogramming.

My theory is that earliest firmware was too sensitive to signal disruption on the coax and some later firmware version allows for more momentary signal loss.

My car was at the dealership since Thursday and I finally got a call today that the Image Processing Module (IPMA) along with all wiring all needs replacement. I kept having the entire vechicle collsion system reboot every day multiple times. Thankfully, I had the premium care extended warranty along with another one. So I only paid $100 out of pocket. May not get the car back till next Monday depending on the parts arrival.

Back to my Toyota Highlander for a week...

I had the coax cable replaced twice and it did not fix it. Ended up with 2 modules being replaced, covered under extended warranty. I was also getting pedestrian alert failures and they replaced a sensor. One dealer claimed they fixed it twice, ended up switching to another dealer that solved all this.
